Speech-Language Pathologist (SLP) – Home Health
Starlight Home Health is seeking a compassionate and dependable Speech-Language Pathologist (SLP) to provide therapy services to patients in their homes. This is a great opportunity for an SLP who enjoys working independently while making a meaningful difference in patients’ safety, communication, and quality of life.
Responsibilities
- Complete speech therapy evaluations and reassessments
- Develop individualized plans of care and treatment goals
- Provide treatment for speech, language, cognitive-communication, and swallowing disorders
- Provide dysphagia assessment, treatment, and education as appropriate
- Educate patients, families, and caregivers
- Communicate changes in condition and recommendations to providers and the interdisciplinary team
- Complete accurate and timely home health documentation
- Coordinate care with nursing, PT, OT, providers, and other members of the care team
Qualifications
- Current Colorado Speech-Language Pathologist license
- Graduate degree in Speech-Language Pathology from an accredited program
- Current CPR/BLS certification
- Valid driver's license, reliable transportation, and willingness to travel to patients' homes
- Strong communication and documentation skills
- Home health experience preferred, but not required
