About the Role:
Star Light Home Health is looking for a passionate and dedicated Physical Therapist (PT) to join our growing Medicare Certified Home Health team. This is a rewarding opportunity to make a real difference in patients' lives by delivering high-quality, one-on-one rehabilitative care right in the comfort of their homes. Responsibilities:
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for home health patients
- Deliver skilled PT services including therapeutic exercise, gait training, balance and fall prevention, and pain management
- Monitor and document patient progress in compliance with Medicare home health guidelines and agency policies
- Collaborate with the interdisciplinary care team including nurses, occupational therapists, and physicians
- Educate patients and caregivers on home exercise programs, safety techniques, and functional independence strategies
- Complete OASIS assessments and maintain accurate, timely clinical documentation
- Participate in care coordination and discharge planning to ensure continuity of care
Requirements:
- Active Colorado Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Experience in home health, geriatrics, or Medicare-certified settings preferred
- Familiarity with OASIS documentation and Medicare home health regulations
- Valid driver's license, reliable transportation, and ability to travel to patient homes in the Grand Junction area
- Strong communication, time management, and critical thinking skills
- Compassionate, self-motivated, and comfortable working independently in the field

Compensation: $90.00 - $115.00 per hour
