Star Light Home Health is looking for a passionate and dedicated Physical Therapist (PT) to join our growing team in Grand Junction, CO. Responsibilities:
- Conduct comprehensive in-home physical therapy evaluations and assessments for Medicare-eligible patients
- Develop and implement individualized treatment plans to restore function, improve mobility, and reduce pain
- Provide therapeutic exercises, manual therapy, and functional training in the home setting
- Monitor and document patient progress, adjusting care plans as needed to meet goals
- Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care
- Educate patients and caregivers on home exercise programs and fall prevention strategies
- Ensure accurate and timely documentation in compliance with Medicare and agency standards
Requirements:
- Active Physical Therapist (PT) license in the state of Colorado
- Graduate of an accredited Physical Therapy program (DPT or MPT)
- Experience in home health, geriatrics, or Medicare-certified settings preferred
- Strong knowledge of Medicare guidelines and home health documentation requirements
- Excellent communication, time management, and organizational skills
- Valid driver's license and reliable transportation for home visits in the Grand Junction area
- Compassionate, self-motivated, and able to work independently in a field-based role

Compensation: $90.00 - $115.00 per hour
